   Independent Study Application Form

Independent Study courses (courses numbered 399) give students an opportunity to pursue a course of study not commonly offered by the College of Commerce.  The student must work with a faculty member and their respective department to gain consent to be enrolled in an Independent Study course.  Independent Study Application Forms should be finalized and in the Commerce Undergraduate Office before the ADD Deadline of the respective term.

Intended Major Application Form

The Intended Major Application Form is your way of notifying the Commerce Undergraduate Program Office what you intend to major in if your intent has changed since applying to College of Commerce.  Processing these forms takes two-three weeks.

Major Declaration Form

The Major Declaration Form is required of all Undergraduate Commerce students.  The major is typically declared at some point in the junior year.  Requirements to declare the majors offered in the College of Commerce can be found in the Commerce section of the on-line course catalog.  Processing these forms takes two-three weeks and notification is sent by US mail.

   Minor Declaration Form

Students in the College of Commerce may pursue a minor in a Commerce field and/or from many of the other Colleges and Schools at DePaul University. Minor requirements can be found in the on-line course catalog.  Please click here  to view form.  Processing these forms takes two-three weeks and notification is sent by e-mail.

   Pass/Fail Application Form

Under the pass/fail option a student who has at least Sophomore standing and who is not on academic probation may register for one pass/fail course each term. A maximum of 20 quarter hours may be taken under the pass/fail option. Grades A through D represent passing performance.  Written permission to use the pass/fail option must be obtained from the student’s academic advisor or from his or her college or school office prior to the beginning of the third week of the quarter. For courses of four weeks or less in duration, approval must be obtained before the second class meeting of the course. The option is limited to no more than one course in any one department.

The pass/fail option may not be used for the following categories of courses:

  • courses taken to meet Liberal Studies requirements;
  • courses taken to meet requirements of a student’s major, minor, or allied field.
  • developmental courses, including those offered through the Writing, Reading, and Computation program and WRD 101 and WRD 102.

If the course is passed, the credit hours earned are entered on the student’s record; the grade is not included in computing the student’s grade point average. If the course is failed, the F grade is recorded on the student’s record and the credit hours attempted are included in computing the student’s grade point average.

Appeal for Course Withdrawal Form

Students considering a late withdrawal must speak with an academic advisor in the Commerce Undergraduate Program Office to discuss the relevant issues related to their need for an exception and the possible effects on their academic career. If warranted, students will receive an Appeal for Course Withdrawal form. This form, completed in its entirety and accompanied by documentation, must be submitted to the Commerce Undergraduate Program Office to be reviewed. Not all petitions are approved. Please note: REQUESTS FROM PREVIOUS ACADEMIC YEARS WILL NOT BE REVIEWED.
